Here's what you need to do:

1. Make an appointment with the MD who treats your diabetes. He or she must be the MD or DO, not a nurse practitioner or physician assistant. Make sure the doctor is documenting your diabetic assessment and plan of treatment and any foot condition related to your diabetes.

2. If you have a podiatrist (foot doctor), make an appointment to see them to discuss your diabetic foot condition. If you don't see a foot doctor, be sure to discuss any foot condition with your MD.

3. Call us to schedule an appointment for your assessment. We will supply your doctor with all the required forms to be completed.
